Original or attested Union Council Death Certificate
Hospital death certificate
Original Membership Document
All past and current medical records
CNIC copies of deceased & beneficiary
FIR/Police Report & Post-Mortem Report (in case of accident)

Free Look Period
If you cancel within 14 working days of receiving your Participant’s Membership Document (PMD), you are entitled to a full refund of your contribution, less medical/clinical exam expenses.
