• Hospital death certificate
• Original Membership Document
• Medical treatment history (if applicable)
• CNIC copies of deceased & beneficiary
• FIR/Police Report & Post-Mortem Report (in case of accident)

Free Look Period

If you cancel within 14 days of receiving your Participant’s Membership Document (PMD), you are entitled to a full refund of your contribution, less medical/clinical exam expenses.
