Plan Term: 5 to 25 years

Maximum Age at Maturity: 75 years

How to Claim

In the unfortunate event of a claim, the beneficiary should contact 5th Pillar Family Takaful at 021-111-786-573 and provide:

• Original Policy Documents
• Claim Forms A, B, C, D
• Medical/Treatment Records with clinical notes
• Death Certificate (Hospital & NADRA issued)
• Attested CNIC copies (life assured & claimant)
• Written intimation with date and cause of death

For Accidental, Murder, or Suicide cases:

• Post Mortem / Autopsy Report (attested)
• Newspaper cutting (attested)
• Police FIR (attested)
• Medico-Legal Report (attested)

Note: Additional documents may be requested depending on the nature of the claim.
