Alfa Insurance Claims

Accidental Death/Permanent Disability Claim:

  1. The Successor/Claimant should contact the designated correspondence address within one month of the Accident, otherwise claim will not be entertained. Average monthly balance of the month preceding the date of incident shall be checked.
  2. Following Documents are required to be submitted by the successor, if the deceased/participant customer is eligible:
    1. Claim Form. (Original, signed & stamped)
    2. Nadra Death Certificate (Computerized).
    3. Police F.I.R.
    4. Copy of CNIC
    5. Hospital Death Certificate.
    6. Statement of the heir / family member dealing with this claim.
    7. Medical record of the treatment of injury given at hospital & residence.
    8. Post Mortem Report.
    9. Newspaper clipping (if available).
    10. Succession Certificate.
    11. Bank Statement from till the date of expiry of account holder.
  3. Upon receiving the above documents, claim will be registered

    ATM & Counter Cash Withdrawal/Loss of Identity Documents Claim:

    1. The Claimant should contact the designated correspondence address within 24 hrs of the incident, otherwise claim will not be entertained. Average monthly balance of the month preceding the date of incident shall be checked.
  4. Following Documents are required to be submit by the participant customer:
    1. Claim Form duly completed and signed by the Customer duly verified by the designated official of the Bank;
    2. I.R. is mandatory for lodging claim under the insurance coverage.
    3. ATM withdrawal slip, if available
    4. Copy of the valid Computerized National Identity Card (CNIC) of the Customer;
    5. Bank statement (from the start of previous month till incident date)
    6. Customer written statement about occurrence of loss.
    7. Upon receiving the above documents, claim will be registered

In case of ATM withdrawal Insurance, the amount mentioned in the Policy Report/FIR registered with Police Station, not exceeding the actual amount withdrawn from ATM shall be considered for claim. The cash withdrawals claims will be settled within fifteen (15) working days and personal accident claims within (30) working days of due communication of the same by the customer, subject to receipt of all the documentation as specified. In case of death, Claim Amount will be paid to the legal heirs as per policy of the Bank Average monthly balance of the month preceding the date of incident shall be checked.
